By completing this form, I certify that the following information is accurate to the best of my knowledge. I understand that purposely providing false information or misusing any funding may result in the termination of my scholarship funding.
IMPORTANT REMINDERS:
- Student must be accepted into the NJFC Scholars Program prior to submitting an Educational Support Request
- Educational supports are approved and awarded on a case-by-case basis. Late fees/fines are not acceptable requests.
- Student must be enrolled in school and maintain at least half-time registration status to receive supports.
- Tuition assistance and/or institution balances are processed prior to educational supports.
- All requests are contingent upon student’s available funding, and funding available through the NJFC Scholars Program.
- Supports are NOT automatically renewed, and must be requested each semester up to $2,500.00.
- Requests may take up to six (6) weeks to be received based on when the student’s school returns required paperwork.
- Educational supports must be related to the current semester in which you are submitting the request. The NJFC Scholars Program cannot assist with educational expenses that occurred prior to the current academic year/current semester.
STIPENDS/GAS CARDS: Pro-Rated Requests

In order to receive the maximum amount of these supports, requests must be received before the start of the semester.
Each week/month the request is not received reduces the amount of funding that can be disbursed.
➢ Fall term requests with weekly amounts will begin decreasing September 8
➢ Spring term requests with weekly amounts will begin decreasing February 9.
❖STUDENT LOAN REDUCTION❖
If you received a credit from your loans, a loan reduction cannot be processed. Loan reductions must be for loans borrowed during the current semester.
If you borrowed federal loans this current semester (subsidized, unsubsidized, Perkins) to help pay for tuition & fees and/or room & board costs, you may be eligible to use NJFC funding toward reducing these loans.

❖TRANSPORTATION❖
- Only students who are living off-campus and are commuting to campus for class and/or tutoring are eligible to submit this educational support. On-campus jobs do not count for this educational support.
- Requests will be calculated at a rate of $.31 per mile.
- Request will be rounded to the nearest $25 increment.
- A processing and shipping fee is charged by the vendor and will be added to the total amount of the request.
- Requests subject to change based on date received by the office.

IMPORTANT NOTES: Students who are receiving an Independent Living stipend from CP&P, residing in a CP&P Resource Home, or residing in a CP&P housing program will not be approved for some or all of the following supports. All requests are subject to change based on the date the request is received by the Scholarship Department.

Dependent Child Support
This support is for NJFC Scholars who have children to help cover caring expenses for their child’s needs. Requests may include a Carseat, Stroller, Crib, Pack & Play, Walker, Diapers, Wipes, or other child-related items.
Request is subject to DCF approval.
To make this request, please email a detailed description of the item(s) you are requesting to your Scholarship Coordinator.
If you have an open CP&P case, please make this request with your caseworker first before requesting through NJFC.

NJFC cannot pay a roommate’s share for lease arrangements.
Required Documents:
- A copy of signed lease with student’s name, the dates of occupancy, and the monthly rent clearly stated.
- A W-9 (tax form) to be completed by the provider and dated in current calendar year. W-9 forms are available from your Scholarship Coordinator or can be accessed here.
If you are seeking a rental reimbursement, a receipt of payment, and a copy of your signed lease must be provided.

Off-Campus Housing Assistance (OCHA)
This assistance is provided when a student is staying with an adult and would like to contribute to the cost of room and board.
